To improve care transitions, GNYHA and the United Hospital Fund have launched the IMPACT (Improve Processes And Care Transitions) to Reduce Readmissions Collaborative. The Collaborative helps hospital and nursing home teams build and strengthen relationships and implement standardized communication and information sharing processes in order to improve care transitions and reduce readmissions.
